With the form below you may request non-identifiable data directly from the SWPHO Public Health Intelligence and Cancer Analysis Teams. NB: Identifiable patient information will only be released to authorised personnel in legitimate organisations. Identifiable information includes data containing postcodes, dates of birth or NHS or hospital numbers, statistics based on fewer than 5 patients or statistics for a population of less than 1000. If your request is for identifiable information please contact SWPHO directly on Tel. 0117 970 6474 and ask for the Public Health Intelligence Team or Cancer Analysis Team depending on the nature of your request.

Please note that the fields highlighted in yellow are mandatory. When you have completed the form click the “Submit Form” button and your request will be emailed to SWPHO. If you specify an email address you will be emailed a copy of your request as a receipt. The receipt may take up to an hour to reach your mailbox.

Most requests will be completed in 7-14 days although more complex analyses may take longer. Your details will be fairly processed and stored, in accordance with the Data Protection Act 1998. Your personal details are used solely for SWPHO internal purposes and will not be passed on to any third parties under any circumstances.
