Postal Data Transfer Form

A data transfer form must accompany all cancer data that is sent to SWCIS. The purpose of the data transfer form is to provide SWCIS with useful information relating to the data.

The SWCIS preferred method of data transfer is via NHS Mail (email addresses that end with "@nhs.net"). To send data via email rather than posting it, please use our Electronic Data Transfer Form

Patient identifiable data must be encrypted to 256 AES standards, e.g. using Winzip 11. Please note that failure to encrypt patient identifiable data will result in SWCIS raising a security incident as per the latest NHS Information Governance directive. The data will subsequently be destroyed.

Follow the instructions below carefully in order to send cancer data to SWCIS:

  • To download the data transfer form: Right click here and select "Save Target As". Save the Microsoft Word file to your c: drive or to a convenient location.
  • Open the file that you have just saved in Microsoft Word.
  • Please note that this file contains important information regarding data and transmission standards.
  • On page 3 of the data transfer document fill out the necessary fields.
  • Once complete, click "Save As" in Microsoft Word and save the form to your c: drive or to a convenient location.
  • Cancer Registration data must be sent to our West Dean (formally Winchester) Office.
  • Audit and Information data must be sent to our Bristol Office.
  • Do not send passwords with the files. We will contact you directly for the password to decrypt the data.
  • Acknowledgement will be sent within one working day.
